Healthy Chicago Equity Zone

The Healthy Chicago Equity Zone (HCEZ) Near South Regional Lead is Greater Auburn Gresham Development Corporation and the following community organizations have served as Community Leads and contributed to the Near South Region Community Health Assessment: Bright Star Community Outreach, Teamwork Englewood, The Kindness Campaign, Chicago Family Health Center, Project H.O.O.D., and UChicago.

Health Equity is the state in which everyone has a fair and just opportunity to attain their highest level of health. Healthy Chicago Equity Zones are advancing health equity by addressing social determinants of health and improving fair and just practices through evidence-based science, programs, policies, and other interventions.

Healthy Chicago 2025 is Chicago’s five-year community health improvement plan that focuses on racial and health equity to meet our goal of reducing the Black-white life expectancy gap. 

The vision for Healthy Chicago 2025 is a city where all people and all communities have power, are free from oppression and are strengthened by equitable access to resources, environments and opportunities that promote optimal health and well-being.
